Kidney Failure Due to Sharp Crystals of Ethylene Glycol


The Ministry of Health (MoH) strongly believes that acute kidney symptoms that have killed up to 143 children and toddlers are caused by contamination of ethylene glycol (EG) and diethylene glycol (DEG) compounds found in syrup drugs. Both are said to turn into oxalic acid, and when it gets into the kidneys, it forms sharp crystals that damage the organ.


The Food and Drug Supervisory Agency (BPOM) has followed up on two industries to be convicted of being caught circulating syrup drug products contaminated with EG and DEG compounds, as explained by the Head of BPOM Penny Lukito on Monday (24/10). Meanwhile, several others are still under investigation.


Children suffering from acute kidney symptoms reported having a fever, up to less and less frequency of urination.


How can ethylene glycol and ethylene glycol be sharp crystals in the kidneys?


Minister of Health (Menkes) Budi Gunadi Sadikin said that ethylene glycol and ethylene glycol compounds damage the kidneys by turning into small sharp crystals in the kidneys. When it enters the body, EG and DEG are processed in metabolic processes, turning into a harmful metabolite called oxalic acid.


This oxalic acid then reacts with calcium and becomes calcium carboxylate. When it enters the kidneys, it will become small sharp crystals that can injure the kidneys from the inside aka kidney failure.


"These chemical compounds, ethyl glycol, diethyl glycol, and polyglycol butyl ether, if they enter our body, our body will metabolize, it turns the chemical compound into oxalic acid. This is dangerous," Budi explained in a press conference.


Why is (oxalic acid) dangerous? The oxalic acid enters the kidneys so that calcium oxalate becomes small sharp-tangy crystals. So that there are small, sharp-sharp crystals in our toddlers, yes, damaged kidneys.

Meanwhile, Professor of the Faculty of Pharmacy, Padjadjaran University, Prof. Mochtaridi, further explained how the crystals finally appeared after ethylene glycol and ethylene glycol entered the body. 

The metabolic end compounds of EG and DEG will accumulate in the kidneys—due to the function of the kidneys as organs of secretion and filtration. The accumulation of such sharp crystalline particles then damages the kidneys , in the same way as the kidney stones.


"So that when (oxalic acid) is in the body, then in the body there is calcium, it reacts to calcium oxalate which will form crystals. And the crystals are deposited in the kidneys on average," explained Muchtaridi.

If it has become a crystal, the oxalate crystal is a needle, yes, it is sharp. Maybe you've heard of kidney stones? Well, if it is already in the kidneys of a small child, it is actually dangerous to expose it.

Muchtaridi asserts these kidney symptoms are more severe in children than in adults.


"It may not be felt when it happens in adults, it may be kidney stones and others, like that. So oxalic acid is categorized as a cellular poison because it enters the cells that cause heart failure, lungs, acidosis, and can even cause depression of the central system due to acidosis," he said.


"Acidosis causes depression of the central nervous system. Well, that's what ethylene glycol and diethylene glycol are known for.
